If Omar Khayyam were writing today, he might refer to typing, clicking and deleting, but his message would be the same – we can’t undo the past.  What we document becomes a record that we can’t change. Yet we are required to document so many things that it can be overwhelming always to keep an eye on the future implications of the records we are (or aren’t) creating.

Think of driving – it’s something we do all the time, but we can train ourselves to drive defensively, thereby reducing the risk of injury from hazards we may or may not be able to see.  So it can be with documenting.
